Nomination deadline is COB NOV 1 

• Excellence in Service - Nominate a single individual for providing superior service during the past year. Please provide specific examples.
• Effective Team - Nominate 2 or more individuals by name who have worked together as a team on a project to achieve a goal that supports the MWR mission. Please provide an example or explanation to support the effectiveness of the teamwork and the outcome of the project or task. 
• Role Model Award - Nominate a single individual for being a role model in the workplace. Anyone who has gone above and beyond to help, lead, mentor or model outstanding employee conduct. Provide specific examples to support the nomination.
